These tools, called “6-Clicks,” are short forms created from the Activity Measure for Post-Acute Care (AM-PAC) instrument, developed by researchers at Boston University. The AM-PAC measures 3 functional domains: basic mobility, daily activities, and applied cognition.

AM-PAC is free for academic research use; there is a charge for clinical/commercial use.
