Palpatine arranged the killing of Shmi Skywalker by the sand people to scar Anakin. Palpatine inserted the fear that Padmé would die in Anakin’s head. That was the way to lure Anakin to the dark side. Finally Palpatine killed Padmé to make Anakin his slave.

25 has provided us with the answer. Anakin’s father is The Emperor. Palpatine manipulated the Midi-chlorians inside of Shmi’s womb to create Anakin. By admitting he learned the secrets to create life with the force before killing his master and therefore would be the only one to create Anakin from Midi-chlorians.

Both R2 and C3P0 have had memories wiped, so they wouldn’t know Ben or Yoda, now the Jedi are just playing ignorant to the fact they know them. R2 has never had his memory wiped. At least not since Phantom Menace.

Palpatine already knew how to influence the midichlorians to create life and save people from dying. So, as Anakin was dying from his wounds on Mustafar, the Emperor siphoned the Living Force from Padmé and brought it to Anakin. Thus, she died as he was reborn as Darth Vader.
