Exit Requirements for Children: Minors (children under 18) who are citizens (including dual citizens) or legal residents of the Dominican Republic, if not accompanied by both parents or legal guardian(s), are required to present official proof of parental consent to travel.

All children must present a valid U.S. passport for international air travel. This rule applies regardless of age, including infants and newborns. If you’re traveling to Canada, minors 15 and younger may cross land or sea borders without a passport, but a certified copy of a birth certificate is necessary.

The ID must readily identify you.

• Valid or expired, undamaged U.S. passport book or passport card.
• In-state, fully valid driver’s license or enhanced driver’s license with photo.
• Certificate of Naturalization.
• Certificate of Citizenship.
• Government employee ID (city, county, state, or federal)

Both parents/guardians must authorize the issuance of the child’s passport. The best way to do this is for both parents/guardians to go with the child in person to apply for the passport.

If a parent refuses to sign a passport application, there aren’t any forms you can fill out to get around it. The law is clear and is designed to protect against international parental child abduction. You will have to pursue legal action and obtain the legal right to obtain [and hold] a passport for your child.

You must have several documents to prove your identity, and one of those is a United States Social Security card. You must provide Social Security number in order to apply for your US passport. If you don’t have you Social Security Card, you should order a replacement card for when you apply for your passport.

The U.S. passport book and U.S. passport card are both accepted as ID for domestic flights. The passport card cannot be used for international air travel, but it is still a great ID to have in your wallet. The Transportation Security Administration (TSA) accepts the passport card as ID for domestic flights.

• An original proof of citizenship document.
• An acceptable photo ID document.
• A photocopy of the front and back of the citizenship document and photo ID document.

Answer: Passports for minors age 15 or younger require the consent of both parents unless one parent has sole legal custody. If your wife has sole legal custody, then your consent would not be necessary. Minors ages 16 and 17 can apply without the presence of their parents. Parental consent may or may not be requested.

Overview. It costs £75.50 to renew or replace your passport if you apply online or £85 if you fill in a paper form. You must be aged 16 or over (or turning 16 in the next 3 weeks) if you want an adult passport. There’s a different process to get a passport for a child.

the Post Office

All passports will be returned by post. Please ensure that you include a stamped, self-addressed envelope with your application (minimum first class large letter stamp). It is not possible to collect passports at the Passport Office.
